Community Health team at Gwinnett, Newton and Rockdale County Health Departments regarding their Diabetes Prevention Program. Recruitment for our August 2022 cohort begins this week! We appreciate the opportunity to serve Conyers Housing tenant(s) through this and our other community programs.
As before, they are targeting residents of Gwinnett, Newton, or Rockdale counties for whom lifestyle change programs or diabetes education is cost-prohibitive or who not have ready access to this support.
This year, we are excited to be able to offer this program virtually AND in-person for the first time since the pandemic began!
